A 54-year-old woman with a known history of COPD has been actively monitored through UTM’s Remote Patient Monitoring (RPM) system for the past two years. Recently, her pulse oximetry readings began trending dangerously low—dropping to 85–87% over a three-day period, well below her typical range of 93–95%.
Upon contact, the patient reported experiencing flu-like symptoms for nearly three weeks. Despite multiple attempts, she was unable to secure a timely appointment with her pulmonologist, who was booked out for another month. In the interim, she sought care at an urgent care facility where she was tested for both influenza and COVID-19, but results were negative, leaving her without a clear diagnosis or treatment plan.
UTM’s software recognized the urgency of her declining oxygen levels and prolonged illness, so I escalated the case to our care team. The case manager promptly sent a direct message to the patient’s pulmonologist, including a summary of her vital signs from the past month. The response was swift: the pulmonologist contacted the patient the same day, prescribed antibiotics and steroids, and scheduled her for an in-office visit within 48 hours.
At the appointment, she was diagnosed with a severe case of influenza—an illness that, if left untreated, could have significantly worsened her COPD. The patient expressed deep appreciation for the intervention, crediting the UTM system and care coordination for facilitating rapid access to her specialist and ensuring she received timely, appropriate treatment. – Jack Love, RN

Within days of enrollment, a patient screened positive for anxiety — something previously undetected. Because of the program’s design, we were able to immediately connect her with the care she needed. Our proactive engagement is helping uncover and address critical health needs beyond blood pressure monitoring.
✅ Acting Fast to Save Lives
One patient’s elevated pulse triggered an alert through our monitoring system. A quick follow-up revealed she was short of breath. Thanks to our care team’s rapid intervention, she was immediately directed to the Emergency Department — potentially averting a major health crisis.

Even in the first weeks of the program, patients began demonstrating remarkable loyalty. One patient, upon receiving her blood pressure cuff and welcome into the program, shared how reassuring it felt to have “a private team” watching over her health. Patients also shared how much they like the blood pressure cuff, prefer it over others, and sometimes travel with them (even if they need to go to the Emergency Department or hospital).
In a healthcare landscape where primary care gaps are widening, our program isn’t just meeting medical needs — it’s restoring trust, engagement, and compliance when traditional systems often fall short.

One patient, caught between insurance coverages after childbirth, shared her fear: “I was scared I’d have to choose between going into debt to take care of myself… or risking my life to save money.” Because of our program, she received cost-effective monitoring at a critical time — offering her both care and peace of mind, without financial hardship. This story is a reminder that good healthcare isn’t just about medical interventions — it’s about removing barriers so people can actually get the care they need.
